Most of those conversations are happening in a vacuum — without any consideration of what is actually going on hormonally for women in midlife.
That is the conversation I want to have today.
If you are in perimenopause or menopause, your metabolic landscape is fundamentally different from what it was in your thirties. Estrogen is shifting or declining. You are losing muscle more easily than you used to. Insulin sensitivity may be changing. Sleep is disrupted, which affects cortisol and hunger hormones. Belly fat appears where it never did before. And the advice to simply eat less and move more is not adequate — not because willpower does not matter, but because the biology driving midlife weight gain is more complex than that.
GLP-1 medications — semaglutide, tirzepatide, retatrutide — work on pathways that go well beyond appetite. They affect insulin, glucagon, gastric emptying, inflammation, and even brain signaling around reward and satiety. That is why so many women feel they finally have traction on weight that would not budge despite doing everything right. But these medications also come with real considerations, especially around muscle loss, and especially in midlife.
